Write down the story, not just the symptom
When did it start? What makes it better or worse? How is it affecting your sleep, work and mood? Patterns help.
List what you've already tried
Medications, therapies, lifestyle changes, what helped, what didn't, and any side effects. It saves time and sharpens the plan.
Be honest about your goals
'I want to sleep through the night' or 'I want to get back to walking the dog' is more useful than 'I want to feel better.'
Bring your questions
Write them down beforehand, it's easy to forget in the moment. No question is too small.
